I/We understand the Board of Foothills Creative Beginnings Pre-school and Kindergarten allows children to participate in field trips which have educational value.

I/We the parent(s) of the child below, consent to my/our child participating in field trips. I/We understand that as part of the kindergarten curriculum, my/our child will participate in community walks on a regular basis. Information about upcoming community walks will be shared with parents through email and/or newsletters.

All field trips requiring motor vehicle transportation and/or those in which we are entering another building will require additional consent. The school will notify parents in writing of the following particulars of any field trip at least three school days prior to the intended date of the excursion.

• destination
• supervision
• dates and times
• transportation plans
• risks and dangers that may be associated with the field trip

This consent and authorization shall be in effect for the current school year only.

All volunteers at Foothills Creative Beginnings Preschool and Kindergarten are required to complete a criminal record/vulnerable sector check. Application for a Police Check can be completed at the local R.C.M.P. station for a cost of $10.
